The Nutritionist and Nurse Diabetes Prevention Program is a structured, lifestyle-focused program grounded in the six pillars of the American College of Lifestyle Medicine, designed to help individuals lower their risk of type 2 diabetes through education, personalized support, and practical habit building strategies.
What the program includes:
- Weekly group and asynchronous sessions are held online, making participation convenient and accessible.
- Guided lifestyle education focused on nutrition, movement, stress, and healthy routines.
- Access to Club 150, a structured movement program designed to help you achieve at least 150 minutes of physical activity each week through simple, realistic exercises.
- Access to Feeding Me, a structured non - medical nutrition therapy program to help you understand 10 key principals for better food choices.
- Ongoing accountability and support through group connection and coaching.
- Progress tracking tools to help monitor weight, activity, and habits over time.
- Goal setting and problem solving strategies to support lasting behavior change.
- A structured curriculum designed to build skills gradually and realistically.
OUR PROGRAM FEATURES

At the heart of the program are live, small group sessions led by a trained Diabetes Lifestyle Coach who is also a Licensed Practical Nurse and Certified Nurse Case Manager. We also offer asynchronous sessions for participants who need flexible, self paced delivery, both options are fully covered by Medicare under the PREVENT DIABETES Act, which permanently authorized virtual and distance learning MDPP delivery through 2029.
You'll meet with your coach and fellow participants in 1-hour sessions held regularly throughout the year, a total of 22 sessions over 12 months, creating consistency, connection, and support.
When you enroll, you'll choose a standing day and time that fits your schedule, so participation becomes part of your routine rather than another obligation.
To support progress tracking, participants log their weight and physical activity every week. You can review your trends and progress over time toward a 5–7% weight loss goal, the clinically proven target that reduces the risk of developing type 2 diabetes by up to 58%. Your lifestyle coach reviews this information every session to provide guidance, encouragement, and accountability throughout the program.
You can access our platform on your computer or smart device for virtual sessions, secure messaging, and health tracking. If you use devices like Apple Health, Fitbit, Garmin, or an Oura Ring, your steps, heart rate, and sleep can automatically sync into your Lifestyle Journal to help you stay on track.
