What is the MDPP and what will you actually do?

The National Diabetes Prevention Program is a 12-month lifestyle change program. Here's what participation looks like:

1 - Sessions 1–16 (Months 1–6)

Weekly group sessions covering nutrition, physical activity, stress, sleep, and behavior change. Facilitated by a trained diabetes lifestyle coach.

2 - Sessions 17–22 (Months 7–12)

Monthly maintenance sessions to reinforce habits, track progress, and prevent backsliding.

3 - Your goal: 5–7% weight loss + 150 min/week activity

These are the clinically proven targets. We'll get you there with structured, realistic support, and not a not a crash plan.

4 - Virtual delivery via Zoom

Every session is facilitated by a clinically trained nurse and certified diabetes lifestyle coach with hands on experience in disease prevention, not an algorithm, and not a one-size-fits-all app.

Your weight, physical activity, and progress are logged and reviewed every single session so nothing slips through the cracks. Over time, that data tells your story and gives you the information needed to adjust your plan and keep you on track toward your goals.

About The Nutritionist and Nurse

The Nutritionist and Nurse is a Houston-based nurse-led clinical prevention practice. We are an approved Medicare MDPP Supplier, a CDC Full Plus Recognized provider, and an American Diabetes Association Partner Organization. Our practice was built specifically to deliver evidence-based diabetes prevention to persons at risk for Type 2 Diabetes.

While we focus on the Medicare population, we serve persons at risk for diabetes type 2. With the clinical rigor of a healthcare practice and the personal attention of a coach, The Nutritionist and Nurse is dedicated to turning a prediabetes diagnosis into a turning point , not a life sentence.

Commonly Asked Questions

Do I have to pay anything?

No. If you have Medicare Part B, this program is fully covered. We handle the billing directly, there is no out-of-pocket cost to you.

How do I know I qualify?

You qualify if you have Medicare Part B and a prediabetes diagnosis confirmed by an A1C, fasting glucose, or glucose tolerance test within the last 12 months. We'll verify your eligibility during your onboarding call.

What if I work during the day?

Sessions are delivered virtually via Zoom. We offer morning and early evening cohorts so you can participate without disrupting your workday.

How long does it take to get started?

Your onboarding call takes about 30 minutes. After that, we confirm your eligibility and enroll you in the next available cohort , typically within 1–2 weeks.

I'm not great with technology. Can I still do this?

Yes. If you can use a smartphone or a computer with a camera, you can participate. We walk every new enrollee through the platform setup during onboarding.
